Spoilers for Agatha All Along and the first two episodes of VisionQuest follow.

How Tommy survives in VisionQuest
Wanda Maximoff’s reality-altering powers turned grief into something dangerous. In WandaVision, she created Billy and Tommy inside the Westview Hex, then seemingly lost them when that artificial world disappeared. Her attempt to find them in another universe drove the events of Doctor Strange in the Multiverse of Madness.

VisionQuest follows White Vision, played by Paul Bettany, as he struggles with the question of who he really is. After Vision died in Avengers: Infinity War, S.W.O.R.D. rebuilt his body as an emotionless weapon. During his battle with the version of Vision created by Wanda, Hex Vision restored his memories. The first two episodes show White Vision attempting a quiet life in Edinburgh while speaking at night with artificial intelligences including J.A.R.V.I.S., F.R.I.D.A.Y. and Ultron inside a private mindscape.
Ultron challenges Vision’s belief that Westview was merely fantasy. By the end of the first episode, Vision must face evidence that Tommy exists beyond the Hex. The bounty hunter Paladin tracks Vision and targets his son as leverage. Without Agatha All Along, the explanation for Tommy’s survival is difficult to follow.

Billy Maximoff found a new body
Set three years after WandaVision, Agatha All Along introduces a mysterious teenager known only as Teen, played by Joe Locke. He helps Agatha Harkness, played by Kathryn Hahn, escape Wanda’s spell and persuades her to form a coven for a journey along the Witches’ Road.

A magical sigil prevents the witches from hearing Teen’s name or discovering his identity. Lilia Calderu, a divination witch played by Patti LuPone, placed the sigil on him years earlier after meeting William Kaplan at his bar mitzvah and seeing a troubling future in his palm. The spell concealed Billy’s identity, even from Lilia herself; it did not resurrect him or deliberately move his soul into William’s body.
When William was 13, he was travelling in a car with his parents as Wanda’s Hex collapsed nearby. The crash killed William at the same moment Billy’s magical world vanished. Billy’s consciousness entered William’s body, but he woke without William’s memories. He could hear the thoughts of the people who believed they were his parents and pretended to recover while investigating the truth.

Billy later describes the experience as dying and returning as something different. Once the sigil is destroyed, Agatha confirms that he is Wanda and Vision’s son living in the body William left behind. She calls William an empty vessel and says Billy instinctively took the chance to survive. That breach of the boundary between life and death makes Billy an abomination in the eyes of Rio Vidal, the literal embodiment of Death.
Billy gives Tommy another life
Billy spends much of the series trying to locate his twin. He cannot remember every detail of their life together, but he remains certain Tommy is still somewhere in the world. Tommy’s consciousness has survived, although it has not yet found a body.

Near the end of the Witches’ Road, Agatha helps Billy search psychically for a suitable vessel. He finds a boy being held underwater after a cruel prank. Billy hesitates because the boy has no loving family waiting for him and because taking the body appears to raise an awful question: would Tommy’s survival mean that Billy caused the stranger’s death?
Agatha tells him that sometimes boys die. Billy then places Tommy’s soul into the drowning boy’s body, giving his brother what Agatha calls “another life”. The series does not clearly show Tommy waking, but creator Jac Schaeffer later confirmed that the transfer was successful. Tommy had not been cast at the time, which is why the sequence was filmed without plainly showing its result.

VisionQuest follows directly from that revelation by introducing the boy who now carries Tommy’s consciousness.
Thomas Shatford is Tommy’s new identity
After Paladin fails to capture Vision with armed mercenaries, he accesses classified S.W.O.R.D. records and searches databases for names and terms connected with Westview, including Vision, Wanda Maximoff, Billy and Tommy. A psychological evaluation recorded in London leads him to Thomas Shatford, played by Ruaridh Mollica.
In the recording, Thomas says that his family and parents were not really his own, while describing his mother as Wanda and his father as a machine. Like William, Thomas was dying when Billy transferred Tommy’s soul into his body. Tommy may also have spent years in a kind of limbo, which could explain why he appears more confused than Billy after awakening.

The first episode presents Thomas as a young London grifter who works with a pickpocket called Lisa. He says he likes becoming a different person each day, but Lisa argues that never being the real you is much the same as being invisible.
That idea becomes painful when Thomas sees news footage of White Vision in Edinburgh. The images trigger flashes of the Westview Hex, the original Vision and his former life. He suffers a panic attack while Lisa repeatedly reminds him that he is Thomas Shatford.
Thomas is not truly Thomas, just as Billy is both Billy and William. Both brothers carry identities and memories that do not fit neatly into their current bodies. Thomas also appears to retain Tommy’s speed, with Lisa casually remarking that he is unusually fast. Tommy was a speedster like his uncle Pietro, suggesting that his powers survived the transfer.

Why VisionQuest depends on Agatha All Along
The result is one of Marvel’s most complicated family histories. Thomas Shatford is not the same physical child that Hex Vision helped raise in Westview. White Vision never personally raised a son, yet he possesses the original Vision’s memories as well as the memories recovered from Hex Vision.
Vision may dismiss Westview as a fantasy created by Wanda, but Tommy’s survival proves that some part of that world endured. Billy survived by taking William Kaplan’s body, then gave Tommy a new life inside the body of Thomas Shatford. That connection is the key to understanding why Tommy is alive when VisionQuest begins.
